Our aim at Livewell Southwest is to support people to stay well in the place and the community they live in, where they can enjoy the best quality of life.
To deliver this we need to be based in and around the local people and communities we serve, and one of the ways we are making this happen is by creating strong networks and partnerships with a wide range of professionals and community groups.
We already have close working relationships with lots of voluntary, charity and community groups, like The Red Cross, Memory Matters, Elder Tree Befriending and Leagues of Friends.
We’re also working closely with our GP colleagues and the health and wellbeing hubs within Plymouth, South Hams and West Devon.
We’ve launched a Partnership Committee to help us to increase the number of community groups we work with so that together we can ensure our services are responsive to the needs of local people, and are increasingly provided within their own community.
We are working to form partnerships with voluntary organisations, education centres, families, advice groups, people who use health and social care services, neighbours and carers, among others.
This way, we can involve local people, voluntary groups and community organisations in the design and planning of the care and support we provide, tailoring it to local need and keeping people safe, well and at home.
