Acute Care at Home team celebrates 2nd birthday

Livewell Southwest’s Acute Care at Home team is celebrating its second birthday today, Thursday 9th February.

Set up in February 2015 by Sue Penrose and a small team of nurses, the Acute Care at Home Team is designed to help prevent unplanned admissions to both acute and community hospitals and reduce the length of inpatient stay, with early supported discharge for patients aged 18 and over who are registered with a Plymouth GP. Operating from 8am – 10pm, 7 days a week, the team care for patients as close to home as possible. This also includes providing care in nursing and residential care settings.  To the end of January 2017, the team have provided 575 episodes of care, saving 4,283 bed days.

The team has continued to grow over the last two years, providing exceptional care and compassions to our patients, with an overall Meridian satisfaction rating of 98%. The team have received a lot of compliments over the two years, including cards and gifts ranging from fresh tomatoes and homemade cakes to wine, biscuits and chocolate! In 2015 the Acute Care at Home Team were nominated for the People’s Choice Award, an Innovation & Development Award and an Outstanding Team Award at the annual People Awards which celebrate the achievements of staff across Livewell Southwest. Attending the Awards Ceremony at Boringdon Golf Club, the team were thrilled to be Highly Commended in the People’s Choice Category. The team’s manager, Sue Penrose, was the winner of the Management category. In 2016 the team was nominated and shortlisted in the People Awards in the Team of the Year category and received a “Commended” Award, as pictured above.


The Acute Care @ Home Team have and continue to work closely with a number of services to provide all round care for each patient, such as Robin Community Assessment Hub and the Acute GP Service, Chest Clinic, GP's, Acute GP Service, Pharmacists, Community Crisis Response Team, the Heart Failure Team, Respiratory Team and Long Term Conditions Team. This enables them to deliver the best possible joined up care to each patient they see.

The team would like to thank all the Livewell Southwest teams and the Community Urgent Care Services Locality who have supported them over their first two years, helping the to provide the best possible care for the people of Plymouth. Here’s to the next year!
