Early Supported Discharge Team (Stroke)


The Livewell Southwest early supported discharge team helps people who have had a stroke to return home sooner from hospital. Individuals who may benefit from this service are referred to Livewell Southwest by acute or community hospitals.

People who use the service agree to work towards rehabilitation goals with the help of physiotherapists, occupational therapists, speech and language therapists and nurses. To qualify, people must be resident or registered with a GP in Plymouth, South Hams & West Devon, or East Cornwall and have a medical management plan that can be safely, and effectively, managed by primary care with appropriate support at home from carers or family. This service is only suitable for people who have recently received a new clinical diagnosis of stroke by a Consultant or via a CT scan (including via a clinical nurse specialist or stroke coordinator).
