he End of Life Care Coordination Centre is the single point of contact for arranging packages of care under NHS Continuing Care for patients who are eligible.
These needs may arise in a planned or urgent way and can occur at any time of the week, day or night, in-hours or out-of-hours.
Our service is staffed Monday – Friday, 9am – 5pm, and is based at Beauchamp Centre, Mount Gould Hospital; at weekends and Bank Holidays a signposting and contingency service is available via Devon Doctors End of Life Team. They will arrange, under the direction of a referring health professional, packages of care with suitable organisations including our in-house Multi-Visit Care Team who provide packages of care to support people to stay at home as their preferred place of care, or will broker out to a domiciliary care agency.
We work closely with the District Nursing Service, the St. Lukes Hospice Crisis Team and our Palliative Clinical Nurse Specialist Services too.
End of Life Champions
Livewell Southwest is very proud to now have a group of 16 End of Life Champions supporting end of life care within our organisation.
The End of Life Champions have completed a specialist End of Life course, “Getting it right at the end of life, right person, right time, right place”, delivered by St Lukes Hospice. The course focused on the necessary competencies for delivering priorities for care of the dying person, commensurate with the individuals role and responsibility. (One Chance to Get It Right: 2014)
End of Life projects being undertaken by Livewell Southwest staff are the “Living well to the very end” improvement programme, sponsored by the Kings Fund and Point of Care Foundation, and the “End of life ambassadors project for homeless people”, delivered by St Lukes Hospice.
Livewell Southwest now has an End of Life Forum in which the champions and other staff interested in end of life care can participate. The aim of the forum is to promote and share best practice around end of life care across all sectors within the organisation.
