Ivybridge/Kingsbridge

Livewell Southwest provides rehabilitation services to people over the age of 18 at The Hayloft, Yealmpton, and South Hams Hospital, Kingsbridge. The rehabilitation services consist of a number of different team members including: physiotherapists; occupational therapists; speech therapists; an intermediate care nurse; support workers, coordinators and administrators.

This rehabilitation service offers responsive treatment, centred on the person’s individual needs; starting with an assessment and leading to home-based care (where appropriate) or treatment on a ward at South Hams Hospital.

We provide intermediate care (crisis intervention), community rehabilitation, ward therapy and group work. Referrals are either received from healthcare professionals or through self-referral.

 

Tavistock

Livewell Southwest provides a community rehabilitation service around the Tavistock Hospital and surrounding areas called the West Devon community team. The Tavistock Community Rehabilitation Team is based at Abbey Rise and offers assessment and treatment for adults living in their own home, residential or nursing homes, or as an inpatient at Tavistock Hospital.

Locality, and deputy locality, managers are responsibility for leading an integrated team of health and social care professionals – including social workers, therapists, community-based and hospital-based matrons and nurses – to ensure people receive the right rehabilitation support, in the right place and at the right time.

The support offered by this services ranges from assessment and case management, community-based rehabilitation for people unable to travel to clinics or surgeries and adult protection from harm. An immediate care service offers urgent interventions to help prevent readmission to hospital, as well as helping to work towards earlier discharge for those already receiving treatment as an inpatient.
