Acute GP Service

The Acute GP Service is based on the Ambulatory Care Unit within Derriford Hospital.

The service offers fellow health professionals advice and access to rapid diagnostic tests in an ambulatory care setting. Patients are able to make informed choices regarding the way in which their care is delivered.

The Acute GP Service has access to other acute services including,Transient Ischemic Attack Clinic (TIA), Neurology and Iron Infusions / Blood Transfusion clinics.

Although the Acute GP Service is based within Derriford Hospital, they also have a presence within the Community Assessment Hub (CAH) – based at Mount Gould Hospital.

There is generally a raft of information that is easily accessible and expert opinions can be sought through the service when advice is required.

Even if an admission cannot be prevented, the service can call upon other services - such as the Acute Care at Home Team or Rapid Response Team to ensure that sufficient care is in place to enable a safe and timely discharge.

65% of all referrals that come via the Acute GP Service result in the patient not being admitted directly to the Medical Assessment Unit – so it is well worth using this service in the management of patient care.
