Alternatives to Admission

Livewell Southwest provides a suite of services that offer credible alternatives to an acute admission to Derriford Hospital and provides:

  • A Single Point of Access: simplified processes so that Derriford, general practice, ambulance services, community teams and social care can make referrals with a single phonecall
  • An interdisciplinary approach for the rapid and comprehensive assessment of patients including medical triage, signposting to appropriate community and voluntary services, referral to HOT clinics, or face to face management of the patient either in the Ambulatory Care Unit (predominantly ambulatory people) or Community Assessment Hub (predominantly frail people)
  • Access to acute diagnostics in the community
  • Acute nursing in the community including nursing clinical assessment and diagnosis, intravenous hydration therapy, intravenous antibiotic therapy, tests and investigations to aid medical diagnosis, monitor treatment and progress of patients, support general practice in the supporting people to remain at home in and out of hours
  • Intermediate care for up to 6 weeks, with a view to support people to maintain or regain independence in their own home or other setting suitable for their needs to avoid an admission

Through an inter-disciplinary, clinically-led approach, all referrals will be triaged ensuring the right service is most appropriately deployed to respond across the Livewell Southwest footprint.
