Rehabilitation Service (West Devon Community Team - Tavistock)

Our services in West Devon are delivered around Tavistock Hospital and its surrounding areas, supported by our Community Team. An integrated health and social care team, we are managed by a Locality and Deputy Locality Manager, and our teams include social workers, therapists, both community and hospital matrons and nurses, working together to ensure that people receive the right care, in the right place, at the right time.

We provide a range of support including assessment and case management, adult protection from harm, healthcare for people unable to travel to surgeries or clinics, and community rehabilitation.

Tavistock Community Rehabilitation Team is based at Abbey Rise and offers assessment and treatment for adults living in their own home, residential or nursing homes, or as an inpatient at Tavistock Hospital. Our team of physiotherapists, occupational therapists, speech and language therapist, remedial instructors, support staff and nurse are supported by administrative staff. Our assessment and treatment of adults with physical disability within the home environment includes group work with Falls Prevention, COPD and Long Term Conditions.

Our Intermediate Care service provides urgent intervention to prevent unnecessary admissions to hospital as well as helping to facilitate earlier discharge.

Patients at Tavistock Hospital receive physiotherapy and occupational therapy input during their hospital stay as part of their ongoing rehabilitation and discharge planning.

Our catchment area is defined by GP surgeries and includes Tavistock, Yelverton and Lifton practices.
