Therapy Team

The Therapy Teams aim to provide multi-disciplinary rehabilitation within the home environment.

We provide holistic, community-based therapy for adults. Service users will be seen at their place of residence (home or care setting) due to being unable to travel or where therapy is best carried out within the home environment.

We provide a multi-disciplinary assessment, treatment and management approach incorporating Occupational Therapists, Speech and Language Therapists and Assistants, Physiotherapists and Therapy Support Workers. We aim to:

  • Provide timely, high quality rehabilitation to meet the changing and continuing needs of service users using a multi-disciplinary and multi-agency integrated approach.
  • Actively promote independence and maximise function by empowering service users and their carers through goal-oriented rehabilitation.
  • Reduce dependency on long term services by maintaining independence and achieving the best possible quality of life.
  • Utilise an integrated process of education, information sharing, assessment, care planning and treatment.
  • Facilitate earlier timely transfer of care from hospital to the most appropriate community setting.
  • Avoid admission to hospital.
  • Support service users with long term conditions.


Referral process:

We are unable to accept self-referrals. If you feel you require our services, please ask your GP or other healthcare professional to refer to us.
