We’re delighted to share that Livewell’s Frequent Users Support and Empowerment (FUSE) team has been awarded ‘Highly Commended’ at the 15th annual HSJ Patient Safety Awards, in the Community Care Initiative of the Year category.
The HSJ Patient Safety Awards recognise the people and projects going above and beyond to keep patients safe and deliver care closer to home, while reducing risks and improving outcomes. This year, there were 456 entries, with just 25 winners chosen by a panel of expert judges.
The FUSE team plays a vital role in supporting people who attend the Emergency Department at University Hospitals Plymouth five or more times in a year. Many of these people feel they have nowhere else to turn, sometimes falling through the cracks of mental and physical health services.
That’s where FUSE steps in. The team offers support and guidance to help people address the issues that matter most to them, involving them in talking therapies to build resilience, and even accompanying them to public spaces to rebuild confidence.
Team members Tristan Merrifield, Isabelle Kingwell, Amber Pollock, Leanne McDonnell, Adam Slater, and Laura Simpkins attended the glitzy ceremony on 15th September.
