We know that most people prefer to be cared for where they live, rather than in hospital.
So we have designed our services to enable people to live independently for as long as possible, wrapping care and support around them. This includes out of hours district nursing and acute care at home, even for people with complex health needs.
Sometimes a hospital stay is unavoidable, but our community teams support people to return home as soon as they are well enough by providing help and care as needed, whether that is physiotherapy, regular visits from the nurses, or occupational health to assess whether any aids or adaptations are needed.
Our community care colleagues work closely with each other, whether they are nurses, paramedics, physiotherapists, occupational therapists, social workers or pharmacists, as well as GP colleagues, to make sure each patient gets the right help, and the need to go back to hospital is prevented.
