We’re thrilled to share that seven of our amazing Livewell colleagues and teams have been shortlisted for the South West Community and Primary Care Nursing Awards, taking place on Thursday 16 October.
These awards celebrate those working in general practice or community settings across the region, recognising innovation and nurse-led initiatives that make a difference to patient care.
Community Nurse Leadership Award finalists
Liz Fox, Manager, Specialist Outreach Recovery Team (SORT)
Liz Fox has been a mental health nurse for 18 years and leads the multi-disciplinary SORT team, supporting people in the community who have a diagnosis of a complex psychosis.
Liz said: “I was thrilled to be nominated for this award and for the work we did as a team to have been recognised. To be shortlisted as a finalist is amazing, it means so much.”
Sonia Green, Practice Lead for Nursing within Adult Frailty Specialist Services (AFSS)
Sonia provides strategic leadership, drives continuous improvement and advocates for nursing development, patient safety and staff support, helping to shape and deliver high-quality nursing care across our community.
Sonia said: “I am honoured to have been nominated and shortlisted for this award, which reflects not only my dedication but also the collective efforts of our teams who strive every day to deliver compassionate, high-quality care in the community. I am proud to represent a service that listens, learns, and leads with integrity.”
Lisa Veale, Manager, West District Nursing Team
Lisa has led the hard-working and resilient West District Nursing Team for almost two years, providing high quality nursing care to people in their own homes.
Lisa said: “If I’m honest, being nominated and shortlisted for this award was a bit of a shock, but I am truly grateful and appreciative that someone has taken the time to give recognition for my leadership and management of the team, who work hard in a challenging demographic.”
Above and Beyond Community Nurse Award finalist
Karina Tobin, Community Sister, North District Nursing Team
Karina goes the extra mile for her patients, providing holistic care in everything from diabetes wound management to end-of-life care.
Karina said: “I was positively shocked to receive this nomination. I am very committed to my role and ensuring that my patient’s needs are met and that their families feel supported.
“I am very proud to be chosen as a finalist, especially doing a job that I really love.”
Children and Young People Award finalist
Sophie Doyne-Ditmas, Student Nurse Associate, CAMHS Eating Disorder Pathway
Sophie supports young people through every stage of their recovery journey, from helping with the running of physical observation clinics to supporting mealtimes in their homes.
Sophie said: “Being nominated and chosen as a finalist for doing something I truly love and feel so passionate about is a huge honour. I am grateful to those who put me forward and to everyone who has supported me throughout my training journey.”
Outstanding “New To” Community/Primary Care Award finalist
Marie Aitkenhead, Clinical Lead, Community Crisis Response Team
Marie supports the management of care for people at risk of hospital admission because of an acute deterioration in their physical health, with the team caring for them at home whenever possible.
Marie said: “I feel honoured to have been nominated for this award and proud to have been chosen as a finalist. The nomination will also ensure the Community Crisis Response Team is recognised for the outstanding work it does daily.”
Working Collaboratively Towards the Future Award Finalist
District Nursing Specialist Practitioners
Chloe Baser is one of our District Nursing Specialist Practitioners, who provide care to people with complex needs and help to reduce hospital admissions and readmissions by providing safe, effective and compassionate care within the community.
Chloe said: “It is a true privilege to have been nominated. I believe that multidisciplinary team practice is key within district nursing to ensure a high standard of care and effective utilisation of resources. Being recognised in this way is especially meaningful as we continue to adapt and evolve within an already in-demand service.”
Shona Cornish, Nursing Professional Lead, said: “I feel honoured to recognise and celebrate the achievements of so many individuals and teams from Livewell at a regional event.
“Our colleagues consistently demonstrate excellence in patient care, innovation, and collaboration. Their commitment to improving health outcomes, even in the face of significant challenges, has set a high standard across the organisation. Whether through implementing new care practices, enhancing patient safety, or supporting staff well-being, their impact has been both measurable and deeply meaningful.
“Their work contributes to a culture of excellence that inspires those around them. I fully support all the nominations and believe they are highly deserving of this recognition.”
