Health Improvement Pathway Plymouth (HIPP)

Supporting people experiencing homelessness and social exclusion

Health Inclusion Pathway Plymouth (HIPP) is a specialist multidisciplinary service that works with people experiencing homelessness, insecure housing and social exclusion who face significant barriers to accessing healthcare.

The service was established in November 2022 to address significant health inequalities experienced by people facing homelessness and multiple health and social complexities, including high rates of hospital attendance, difficulties accessing healthcare and poorer health outcomes..

HIPP is jointly delivered by Adelaide Street Surgery, Plymouth Alliance and Livewell Southwest, with consultation and support from Pathway UK.

Our aim

HIPP works across hospital and community settings to improve health outcomes and reduce health inequalities.

The service focuses on three key areas:

Admission Avoidance

Supporting people to access healthcare and social support in the community wherever clinically appropriate, helping to prevent avoidable hospital admissions.

Inpatient Support

Providing specialist support during hospital admissions, helping people engage with treatment and reducing the risk of self-discharge. The team has a dedicated duty worker based at Derriford Hospital Monday to Friday.

Safe Discharge

Helping people leave hospital safely by coordinating accommodation, ongoing healthcare, community services and follow-up support.

What we do

HIPP provides coordinated, person-centred support for people with complex physical health, mental health and social care needs.

Support may include:
