What are Integrated Neighbourhood Teams?
Integrated Neighbourhood Teams (INTs) are groups of health, social care and community professionals who work together to support the health and wellbeing of people living within a local neighbourhood.
Rather than services working separately, Integrated Neighbourhood Teams bring together professionals from different organisations, such as:
- General Practice (GPs)
- Community nursing teams
- Therapists and rehabilitation services
- Mental health services
- Social care teams
- Voluntary, community and social enterprise organisations
- Pharmacy teams
- Housing and wider community support services
By working as one team around the needs of local people, Integrated Neighbourhood Teams aim to provide more joined-up, personalised care and support.
What does this mean for local people?
Integrated Neighbourhood Teams help people to:
- Access support closer to home.
- Tell their story once, rather than to multiple services.
- Receive more coordinated care from professionals who work together.
- Get help earlier, preventing problems from becoming more serious.
- Maintain independence and wellbeing for as long as possible.
- Receive support that reflects what matters most to them and their families.
How do Integrated Neighbourhood Teams work?
Teams regularly meet to discuss how best to support people with complex or changing needs. They share expertise, coordinate care plans and work closely with local communities to identify opportunities to improve health and wellbeing.
The focus is not only on treating illness but also on:
- Prevention and early intervention.
- Supporting people with long-term conditions.
- Helping people recover after illness or hospital stays.
- Reducing health inequalities.
- Building stronger, healthier communities.
Our Vision
Integrated Neighbourhood Teams are about delivering the right care, from the right professional, at the right time, in the right place. By bringing services together around local communities, they can provide more proactive, person-centred support and help people to live healthier, more independent lives.
In simple terms: Integrated Neighbourhood Teams are local partnerships of health, care and community organisations working together to support the needs of people and communities, ensuring care is more joined-up, proactive and focused on what matters most to each individual.
