Across Plymouth, our dedicated staff work within local neighbourhoods to provide care, support and advice to the people who need it most.
We bring together a wide range of professionals, working as one team to deliver joined-up, personalised support.
By working closely with each other, with our communities, with primary care and our wider system partners, we make it easier for people to access the right help at the right time.
Teams like our District Nurses, Community Therapy Teams, Community Mental Health Teams and many more have been designed to support and care for people where they live, rather than in hospital. They provide help and care as needed, whether that is physiotherapy, regular visits from the nurses, or occupational health to assess whether any aids or adaptations are needed.

Improving Lives Plymouth is a long-established local charity supporting people to improve their physical and mental wellbeing. They provide a wide range of services including information and advice, support for long-term conditions, carers’ services, and inclusive wellbeing activities across the city.
Elder Tree Befriending Service
Elder Tree supports people aged 50 and over to stay active, connected and independent. Through befriending, social groups and physical activity programmes, they help reduce loneliness and improve wellbeing for older people across Plymouth.
Plymouth Octopus Project (POP)

POP supports and connects Plymouth’s voluntary and community organisations. They provide funding advice, training, and networking opportunities, helping grassroots organisations thrive and collaborate to improve outcomes for local communities.
Shekinah
Shekinah works with people experiencing homelessness, substance misuse and social exclusion, providing support, training and opportunities to help people move towards independence.

Primary Care (GP practices)
Primary care is often the first point of contact for people accessing healthcare. GP practices and primary care teams provide a wide range of services, including diagnosis and treatment of illnesses, management of long-term conditions, preventative care, and referrals to specialist services when needed.
University Hospital Plymouth NHS Trust (UHP)
University Hospitals Plymouth NHS Trust provides specialist and acute hospital services for people across Plymouth, South West Devon and East Cornwall. As the region’s largest hospital trust, UHP delivers a wide range of services, including emergency care, planned treatment, specialist clinics, diagnostics, and maternity services.
Plymouth City Council
We work in partnership with Plymouth City Council to deliver integrated health and social care services and improve outcomes for local people through a joined-up, place-based approach.
One Devon
One Devon is a collaboration of the NHS and local councils, as well as a wide range of other organisations like the voluntary sector, who are working together to improve the lives of people in Devon.
Livewell Foundation
The Livewell Foundation supports projects and initiatives that improve health and wellbeing, reduce social isolation, and prevent ill health across Plymouth and South West Devon by providing funding to local organisations and community groups.
