Information for Weight Management Patients

About the Weight Management Service

 

Maintaining a healthy weight is one of the ways to support our overall health and well-being, but it doesn’t always feel easy. Livewell Southwest has a team of specialists who provide free advice and support to local people about how to improve their diet and achieve a healthy weight for them.

If you are living with obesity and would like support, the Weight Management service may be able to help. To access the program, please speak to your GP who will ensure you meet our referral criteria, who will then be able to refer you.

Unfortunately, there is a significant wait to be seen by our service currently, which we are working towards improving.

If your referral is accepted, once you are at the top of the waiting list, you will receive an invitation to attend one of our face-to-face induction sessions. If you choose to opt in to the programme, you will be offered a face-to-face initial assessment appointment to discuss your needs and next steps. If appropriate, you will then be given the opportunity to attend an educational programme. After this programme, you will be given the chance to have additional support from our team of Specialist Practitioners.

The specialist team, which includes Nurses, Dietitians, Health Improvement Practitioners, Physiotherapists, a Clinical Psychology team and Consultants, will support you with your continued lifestyle change. We take a holistic approach; encouraging activity, changing eating habits, goal setting and looking at how thinking or feeling affects our eating.

If you are interested in being referred for support from the weight management service, please contact your GP Surgery, Consultant or Specialist Nurse.

If you do not meet the criteria for the Weight Management Service, you may be able to ask your GP for a referral to other services for support such as:

  • NHS Diabetes Prevention Programme
  • NHS Pathway to remission Programme
  • NHS Talking Therapies
  • Eating Disorder Service
  • NHS Digital Weight Management Programme

This service is not available to people under the age of 18, those who are pregnant, people who cannot attend any face-to-face appointments, people who are in a care setting where food is provided, or patients who have self-funded bariatric surgery.
