A dementia review helps us understand your care and support needs.
It takes around an hour and a half.
What happens in a dementia review?
A support worker will visit you and your carers every six months.
The support worker will talk to you and your carers.
They will ask about lots of different things.
They will write down what is going well.
They will write down things you or your carers need help with.
What happens after a dementia review?
The support worker will talk about this in a meeting with other professionals involved in your care.
They will write to you about what they found out.
