Livewell Southwest action plan following an independent investigation into the care and treatment of a mental health service user Mr P in Plymouth
NHS England has published the findings of a report by Niche Patient Safety (who were commissioned by NHS England) to undertake an independent investigation of a mental health homicide review into the care and treatment of Mr P, who was a service-user under the care of Livewell Southwest.
