Heart Failure

The Community Cardiac Team helps people to manage their Heart Failure symptoms better and get the most out of their treatment.

Once referred, the patient will be triaged against Livewell Southwest’s Heart Failure Referral Criteria. If suitable, a telephone call will be made to discuss their current condition and an assessment will be arranged. During the first appointment, the nurse will assess the patient’s individual needs and give information on issues that are relevant, such as medication, how to control  symptoms and manage their condition, diet and exercise plans and ways to reduce stress. The Nurse will also work with the patient, their GP and, if needed, a Hospital Consultant, to work out the optimal medications needed to take to keep healthy.

We also offer supervised exercise programmes, provided by dedicated Cardiac Rehabilitation Nurses and Specialist Exercise Instructors, which help people to get moving in a structured and well-supported way over a six week course. This also offers people the chance to meet others who have experienced the same cardiac event, or procedure.

We currently run the exercise sessions at YMCA Sports Centre in Honicknowle, Plymouth, The Youth Hut in Horrabridge and the Sports Pavilion, Queen Elizabeth II Recreation Field, Modbury.
