How can I join the course?

Any healthcare professional can refer you,  e.g. GP, Practice Nurse, Physiotherapist. Some pharmacies are also able to refer.

What should I expect?

Following referral you will be sent a letter, you will need to respond to this letter to ‘opt in’.

You will then be telephoned by a Respiratory Nurse, who will ask you some questions about your condition, any other medical issues you may have and how your condition is affecting your everyday life. Also if you are wishing to take part in the Face to Face classes or the virtual classes. This telephone appointment may take approx. 30 minutes

Following this telephone assessment a face to face assessment at The Elm Centre will be arranged.  At this appointment we will take some basic observations- heart rate, blood pressure etc and you will speak to a Physiotherapist. Following that you will take part in a walking test to assess your current level of ability. This appointment will take approximately 30 minutes.

You will then arrange a start date and attend 6 consecutive weeks of Exercise and Education sessions (Mondays and Thursdays)

Following completion you will have a discharge assessment, during which you will repeat your walking test and will discuss your ongoing plans for integrating regular exercise into your everyday life.
