Referrals to the Livewell Southwest Community Crisis Response Team can be made by a health or social care professional who has been in contact with an individual in the last 24 hours. Please note that the team is a physical crisis team, and not a mental health crisis team. For mental health crisis team, click here.
Livewell Southwest’s community crisis response team aims to prevent unnecessary hospital or residential care admissions, promote and support independence, identify any long term needs and support people who are in crisis to remain in their usual place of residence.
The service is made up of administrative team and coordinators, paramedics, community support workers, occupational therapists, a physiotherapist, registered general nurses and mental health nurses, social workers and community care workers.
The service offers combinations of short-term packages of domiciliary care, physiotherapy, occupational therapy and nursing input, reablement services and equipment to help people to maintain their independence. Typically, the service supports people for six weeks or less and where people will benefit from longer-term support, the team will assist in the referral or transfer of their care to a responsible community team.
