Long Term Conditions

The Livewell Southwest long-term conditions (LTC) team is a city-wide service for Plymouth, with each GP surgery having a representative on the team.

This service is designed to help meet the needs of people with complex, long-term health needs. The aim is to provide joined up, holistic care by working closely with multiple agencies to help protect people’s independence as much as possible, empower them to make decisions about their care and promote a good quality of life.

Central to the success of this service is keeping people out of hospital – or shortening their length of stay if they are admitted – and instead treating and supporting people in the preferred place of care.

Community matrons play a key role in delivering the long-term conditions service, in particular for older people with multiple health conditions. They proactively help to identify people who may benefit from this type of case management support, undertake clinical assessments, intervene in crisis situations and work with families and individuals to promote effective self-management.
