This specialist team provides mental health support, assessment and care planning advice to all areas of Derriford hospital in respect of older adults who may be experiencing a change in their mental state or who have existing mental health needs which require ongoing support whilst they are in hospital.
Referrals are accepted from members of the ward multidisciplinary team and also older adults themselves or their families if they have concerns about their own or family members mental state whilst in hospital, although we would encourage people to first talk to the ward staff looking after them.
