Older People’s Mental Health Functional Community Team

The Older People’s Mental Health (OPMH) Functional Community Team is a service for people over the age of 65 with a functional mental health problem, e.g. anxiety, bipolar disorder, depression or schizophrenia, that cannot be managed by your GP alone.

The team comprises mental health nurses, occupational therapists, psychiatrists, a psychologist and support workers.

Currently the service is available Monday – Friday, 9am – 5pm, all year round except on Bank Holidays. Referrals should be made via your GP.

 

Occupational Therapy

Our Occupational Therapists will assess those referred whose occupational needs have been affected by their mental and physical health. A care plan will be agreed to improve their independence, recovery and engagement in occupational activities.

Assessment and intervention to improve their independence in every day activities will take place in their own home, environment or community setting.

The Occupational Therapists may need to assess for and prescribe minor adaptations and issue equipment to help improve safety, facilitate independence, improve self confidence and to access their local community. They provide support to access community groups and to attend specific therapeutic groups, facilitated by Livewell Southwest staff.
