Livewell Southwest offers a Specialist Outreach Recovery Team (SORT) that works with adults who have long and enduring mental health issues, primarily schizophrenia, and who find it difficult to engage with other teams and services. SORT also work with people from the point of admission to our adult mental health recovery inpatient units, Syrena and Greenfields, providing intensive transition work from hospital to home in order to support people who may have spent a long time in hospital
SORT is provided by a multi-disciplinary team – a collection of professionals with different skills in mental and physical health who work together. This includes psychologists and psychiatrists, specialist nurses, occupational therapists and assistant practitioners as well as links with a housing support worker and adult mental health social services.
Our therapeutic model is centred around the individual and is recovery-focused. The aim of Livewell Southwest’s Specialist Outreach Recover Team is to enable the people we support to become as independent in the community as possible, helping them to improve their quality of life at a pace that suits their individual needs.
The people we support are often facing complex psychological, social and emotional issues, alongside medical issues. We offer services to help build – with input from them and their carers or loved ones – bespoke plans that can be delivered proactively, and within the community setting. Our aim is to reduce the frequency of, or need for, hospital admissions and work collaboratively with the individual towards recovery.
Our service offers support on a range of issues, from the challenges of using medication properly, symptom management and education, through to drug, alcohol and other substance misuse or ways to identify if a relapse looks likely.
We also work with other teams to provide support packages to help with matters such as housing, community activities and volunteer work. The team also offers help and support to family and carers, through a Carers Group, as well as a range of socially inclusive activities.
